Texas Consumer Notices
Patient Rights and Records
Effective September 1, 2025
Pursuant to Texas Health and Safety Code Section 181.105 and Texas House Bill 4224
This notice is provided in accordance with state law regarding Protected Health Information (PHI) and counseling records.
Your Rights
As a client, you have the right to:
Access and obtain a copy of your counseling records (paper or electronic), as permitted by law
Request that your records be sent to a third party in writing
Request amendment or correction of your records
Receive an accounting of certain disclosures of your PHI
Request restrictions on certain uses and disclosures of your information
Withdraw consent for electronic disclosure, when permitted by law
Be informed about how your health information is used, stored, and transmitted
Please note: Access to certain portions of your record may be limited if release would be harmful or otherwise restricted by law.
To request your records, please email info@madewellcounselingandwellness.com with your request. Some disclosures may require a signed Release of Information, which will be provided to you through the secure client portal if needed. Requests for records may take up to 30 business days to process.
Fees: You may request copies of your complete record for a $50 fee.
If you prefer, a summary of services can be provided. Time spent preparing records, summaries, or other administrative services is billed at $200 per hour, in 30-minute increments.
Electronic Records
Your Protected Health Information may be stored or transmitted electronically for purposes including:
Treatment and coordination of care
Insurance billing and payment processing
Electronic health record (EHR) systems
Secure electronic communication
Telehealth services
This practice maintains administrative, technical, and physical safeguards to protect the privacy and security of your information in compliance with Texas and federal law.
Notice Concerning Complaints
The Texas Behavioral Health Executive Council investigates and prosecutes professional misconduct committed by Licensed Professional Counselors in Texas.

The Council investigates complaints alleging violations of the law, including but not limited to professional misconduct, unprofessional conduct, or violations of state regulations governing the practice of counseling.
